Prospective Parents

For prospective parents or guardians, please review the parent information. If you are interested in enrolling, please download our Registration Packet and return completed forms to the school. Completed forms can be mailed, emailed, or dropped off at the school office.
In addition, parents will be expected to bring in birth certificates and provide up-to-date immunization records. Furthermore, parents driving on field trips will be required to provide a valid driver’s license and proof of insurance.

Vaccination Information

The state mandates that we share school immunization/exemption rates. This year (2017), our adjusted enrollment was 53 students, of which 41 claimed nonmedical exemptions for a total of 77.3%. In comparison, for January 2017, our adjusted enrollment was 53 students, of which 37 claimed nonmedical exemptions for a total of 69.8%. For January 2016, our adjusted enrollment was 39 students, of which 23 claimed non medical exemptions for a total of 59%. For January 2016, our adjusted enrollment was 48 students, of which 25 claimed non medical exemptions for a total of 52%.

For parents who wish to claim a non-medical exemption, please review this information from the Oregon Health Authority. You can either meet with a health care practitioner or watch an online module. The online module can be found here, though please note you will need to print a certificate at the end, so you will need access to the printer while you view the module. (Also, please be careful as you near the end: there is a prominent “next” button on the screen where the certificate is, and if you click the button, you may not be able to print the certificate.)
